Steps In The Process
STEP 1: Schedule an appointment online. Referrals are not required.
STEP 2: Complete online questionnaire. Must be completed prior to visit, or visit will need to be rescheduled.
STEP 3: Complete scheduled appointment with doctor.
STEP 4: Visit notes as well as any letter of medical necessity for needed services will be provided within 7 business days via the patient portal.
Telehealth Appointments
Joshua Health is an innovative multi-state telemedicine practice. Our providers are currently licensed and able to provide virtual consultations for patients physically located in many states. Appointments are made online. Please click on the link “BOOK APPOINTMENT” and select the desired date and time.
We understand occasionally circumstances prevent you from keeping your scheduled appointment. If possible, please notify our office in advance if you are running late or unable to make your appointment. We will do our best to reschedule you promptly. There is a fee for cancellations or Missed ("no show") appointments. 15 minutes or more late is considered a missed appointment. Cancellations must be done no later than 48 hours prior to appointment to avoid fees.
In case of an emergency, call 911 or go to the nearest emergency room.
Insurance and Billing Policy
Payment in full is expected from you at the time of your visit. We can supply you with an invoice which you can submit to your insurance company for reimbursement.
We are pending insurance credentialing with the major insurance commercial carriers - Aetna, Cigna, Blue Cross/Blue Shield, and United Healthcare. If you have questions regarding whether we take your insurance please contact our office. Insurance will be verified prior to confirmation of your appointment.
Payment Options
Billing and Payment is conducted through the “Patient Portal”. We also accept credit cards, HSA, FSA, and Care credit.
