Steps In The Process
STEP 1: Schedule an appointment online. Referrals are not required.
STEP 2: Complete online questionnaire. Must be completed prior to visit, or visit will need to be rescheduled.
STEP 3: Complete scheduled appointment with doctor.
STEP 4: Visit notes as well as any letter of medical necessity for needed services will be provided within 7 business days via the patient portal.
Telehealth Appointments
Joshua Health is an innovative multi-state telemedicine practice with no in-person office available. Our Doctors are currently licensed and able to provide virtual consultations for patients physically located in any State colored purple in the image below. Some states are still pending. IF THE STATE WHERE YOU ARE PHYSICALLY LOCATED IS NOT COLORED PURPLE, THEN WE DO NOT HAVE A DOCTOR LICENSED IN THAT STATE, IN WHICH CASE PLEASE DO NOT BOOK AN APPOINTMENT. Appointments are made online. Please click on the link “BOOK APPOINTMENT” and select the desired date and time.
We understand occasionally circumstances prevent you from keeping your scheduled appointment. If possible, please notify our office in advance if you are running late or unable to make your appointment. We will do our best to reschedule you promptly. There is a fee for cancellations or Missed ("no show") appointments. 15 minutes or more late is considered a missed appointment. Cancellations must be done no later than 48 hours prior to appointment to avoid fee.
In case of emergency, call 911 or go to the nearest emergency room.
Insurance and Billing Policy
Payment in full is expected from you at the time of your visit. We can supply you with an invoice which you can submit to your insurance company for reimbursement.
We are pending insurance credentialling. If you have question regarding whether we take your insurance please contact our office. Insurance will be verified prior to confirmation of your appointment.
